Gastric Emptying in Diabetic Patients and Effect of Cisapride

Abstract
Background :
@EN Diabetic gastroparesis is one kind of autonomic nervous dysfunction in diabetic patients Cisapride is a prokinetic agent which enhance gastrointestinal motility, and improve diabetic gastroparesis.
@ES Methods:
@EN Gastric emptying of technetium labeled solid meals was studied in 15 diagetic patients with gastrointestinal symptoms in the absence of structural and mucosal abnormalities, and 12 normal subjects. We svaluated the relationship between a
variety of
clinical symptoms and gastric emptying time, and studied the effects of cisapride administration on gastric emptying and gastrointestinal symptoms.
@ES Results
@EN 1) Baseline mean half gastric emptying time for solid meal was significantly delayed in 15 patients (110.3¡¾¡¾29.1 min) compared with control group (72.1¡¾11.1 min). Eleven patients were abnormaly delayed in their half gastric emptying time.
2) Fifteen patients had various gastrointestinal symptoms (epigastric pain and discomfort, early satiety, indigestion, bloating, nausea, and vomiting), and there was a tendency to increase in half gastric emptying time in patients with severe
gastrointestinal symptoms.
3) Half gastric emptying time was slightly delayed in patients with diabetic peripheral neuropathy or autonomic neuropathy compared with patients without diabetic neuropathy.
4) After administration of cisapride (10§· t.i.d. for 4 weeks), mean half gastric emptying time for solid meal was significantly improved compared with baseline mean half gastric emptying time (110.3¡¾29.1 vs. 77.5¡¾24.4 min, p<0.01), and
gastrointestinal symptoms were also significantly improved.
@ES Conclusion :
@EN Gastric emptying time of solid meal was delayed in diabetic patients with gastrointestinal symptoms. Administration of cisapride improved both gastric emptying and gastrointestinal symptoms.
